Polysubstance Use Assessment Tool (PAT)

The polysubstance assessment tool is a novel measure to capture polysubstance use (the use of more than one substance at the same time). It was created as an interviewer-administered version via REDCap as well as an app-based self-administered version. The interviewer-administered tool captures high-risk polysubstance use (based on 6 high risk substances) while the app version captures all combinations. This license request is only for the interviewer-administered version.

**Please Note: For Research and Academic Use agreements, an institutional signatory is required to complete the process. An Institutional Signatory has the ability to sign on behalf of an institution for legal and financial matters (often a member of your legal, technology transfer, or leadership staff). Before you begin the process, we advise knowing who the appropriate signatory is for your institution (including their name and email).
