The Adult ADHD Self-Report Screener, symptoms checklist, and proprietary scoring rules (ASRS DSM-5) is a self-report questionnaire designed to assess Attention Deficit Hyperactivity Disorder (ADHD) symptoms in adults (18+).

The Adult ADHD Self-Report Scale is an instrument updated to DSM-5 criteria; the screener is contains six questions that were found to be the most predictive of symptoms consistent with ADHD. These six questions are also Part A of the ASRS-5 Symptom Checklist. Part B of the Symptom Checklist contains the remaining twelve questions.
